Configuring Drop Down Options:

1) Choose LookupCode "PtContactStatus", then Attribute is FollowUpType, then Value is Survey: the attribute should be assigned to "EPIC-26 Survey" and "RAND Survey".

2) Choose LookupCode is ProcName, Attribute is ModuleDefault, Value is FollowUp: the attribute should be assigned to "RP" and whatever other procedures you may use to track follow up patients (I guess brachytherapy now).
